HealthScores.ai is now generally available to people with Medicare, providing a simpler way to connect supported medical records, organize important health information, and securely share that information with caregivers and healthcare providers.


Health information is often spread across multiple healthcare organizations, patient portals, documents, and applications. HealthScores.ai helps bring that information into one patient-controlled experience, making it easier to understand your health, prepare for appointments, and stay connected with the people involved in your care.


The trial provides access to the HealthScores.ai app. The optional HealthScores.ai QR sleeve is a separate physical accessory, is sold separately, and is not required to use the app.

Supporting the CMS Kill the Clipboard Initiative


HealthScores.ai supports the goals of the Centers for Medicare & Medicaid Services’ Kill the Clipboard initiative: reducing repetitive paperwork and helping patients electronically share their health information with healthcare providers.


With HealthScores.ai, members can organize important information such as medications, allergies, health conditions, test results, medical visits, and insurance information from supported sources. Members can then generate a secure QR code or SMART Health Link to share selected information when checking in for an appointment.


The goal is simple: spend less time recalling and rewriting the same medical history and more time focused on the care you need.

HealthScores.ai App Trial


The promotional trial provides eligible participants with access to the HealthScores.ai mobile application. The app allows members to organize supported medical information, view their Health Score, collaborate through a Care Circle, and generate QR codes for sharing.


Trial availability is limited, and eligibility and redemption requirements apply.


Optional HealthScores.ai QR Sleeve


The HealthScores.ai sleeve is a separate physical accessory designed to attach to a compatible watchband and display a member’s emergency QR code.


The sleeve:

  • Is sold separately
  • Is not included with the app trial
  • Is not required to use HealthScores.ai
  • Does not include a smartwatch or watchband
  • May be ordered separately by members who want convenient, wearable access to their emergency QR code
