One number

What is a health score?

A health score is a 0 to 1000 number from HealthScores.ai that turns your medical history into something your whole family can understand. Most people can't answer a simple question: how am I doing? They have a stack of lab results, a few numbers they remember from their last visit, and a general sense that they should be walking more. Like a credit score turns your accounts and payment records into an easy-to-read number, a health score brings your health data together into one number you can watch and move by acting on recommended actions.

The score runs from 0 to 1000. Higher means more of the factors in your profile are trending the right way. Lower doesn't mean you're sick. It means there are things worth paying attention to, and the app tells you which ones.

What's inside

The HealthScores behind your HealthScore

Your health score is made of smaller HealthScores. A baseline band, worth up to 250 points, reflects what you can't change. An actionable band, worth up to 750, reflects what you can change, across five pillars worth 150 each: cardiometabolic, lifestyle, mental and social, preventive care, and engagement and adherence.

That split is why a lower score doesn't mean you're sick, and why the number moves when you act.

How it works

From your profile to a score you can act on in four steps

01

Build your Health Profile and see your first score.

Add conditions, medications, allergies, care history, and your records. Your health score appears from what you've entered, built from smaller HealthScores for each part of your health.

02

Connect your health apps.

Use the mobile app to sync activity, sleep, heart rate, and other vitals from the health apps and wearables you already use, and your score sharpens with the new data.

03

See what's moving it.

Your HealthScores show which parts of your health are pulling the total up or down, so you know where to start.

04

Act on what you see.

Recommended actions cover the everyday things you can do to lift your score, like scheduling an appointment or being more active. Notifications flag changes worth a look, like a new lab result or a heart rate reading outside your usual range, so you and your Care Circle can stay ahead of them.

The app never guesses from data you haven't shared. Without a wearable, your score is built from your records and entries alone. Connecting one gives it more to work with.

Why it matters

How a score helps prevent avoidable ER visits

The trips that could have been avoided often start with something noticed too late. A refill that ran out on a Friday. A blood pressure reading that climbed for two weeks with nobody watching. A follow-up appointment that never got scheduled. A score that moves, and recommended actions that arrive on time mean someone is noticing. That's how HealthScores.ai helps prevent avoidable ER visits.

If you're caring for a parent or spouse, their score is one of the first things you'll check. With their permission, you see it, watch how it changes, and see the same recommended actions they do. A phone call goes from "how are you feeling?" to "did you get that refill?"

What it isn't

A picture of your health, not a medical opinion

What it is: a summary. Your health score reflects what's in your profile and your connected devices, so you and your Care Circle can see patterns and trends over time and act on the ones you can change.

What it is not: a medical opinion. It doesn't tell you what's wrong or what to do about it medically. When something in it concerns you, that's a question for your care team.

HealthScores questions

  • What is a good health score?

Higher means more of your health is trending the right way. What matters most is whether your score is moving up and which recommended actions will move it next.

  • How often does my HealthScores update?

Whenever your records change, you add something, or a connected health app syncs.

  • Can my doctor see my HealthScores?

Only if you share it. You can include it in the information you make visible through your SMART Health Link, or show it on your phone during a visit.

  • Can my Care Circle see my score?

Only the people you allow. Permissions are set person by person.

  • Can I raise my HealthScores?

Yes. Schedule that overdue screening, and the number moves. Take a walk, and it moves. A new lab result moves it too. The score gives you credit for what you do, not just what your chart says.
