Wear the sleeve as directed so it is easier to notice. In an emergency, personnel with a compatible connected device can scan the QR code and view information you made available to share.

Do I need to be a HealthScores.ai member?
Yes. The sleeve is an additional offering for HealthScores.ai members because its QR code connects to information managed through the member's HealthScores.ai profile.
Is my medication information stored on the sleeve?
No. The sleeve displays a QR code that connects to the emergency information you maintain through HealthScores.ai.
Do I need my phone with me?
The member's phone does not need to be unlocked or presented for the QR code to be visible. A non-EMS person can scan the code to get the basic information. To access a more in-depth listing of your medical information (devices, procedures, etc.), you will need your phone so the EMS practitioner can geo-locate with your HealthScores.ai app to access that information by IAL2 identity.
Can I update my medication information?
Yes. Update your medication information through HealthScores.ai. Because the sleeve connects to your emergency profile, you do not need a new sleeve each time your medication list changes.
Does the sleeve guarantee that emergency personnel will scan it?
No. The sleeve makes the QR code more visible and accessible, but HealthScores.ai cannot guarantee emergency personnel will notice or scan it. Use can depend on the situation, available technology, and local emergency-response procedures. You might want to notify your local fire department that your family has a HealthScores.ai sleeve to help in an emergency situation, benefiting everyone if that emergency happens at home.
Does it replace a medical alert service or traditional medical ID?
No. The sleeve is an additional way to make member-selected information available. It is not a substitute for calling 911, professional medical care, a monitored medical alert service, or identification recommended by a healthcare professional.
