When you can't speak for yourself

What is the HealthScores.ai sleeve?

The HealthScores.ai sleeve is a QR code you wear on your watch band. Scanned with any phone camera, it opens your Emergency Profile, so the people helping you can see what you chose to share.

When paramedics arrive, they work with what they can see and what you can tell them. If you're unconscious, confused, or unable to answer, that isn't much. The sleeve gives them your allergies and alerts, your care team, and who to call, as context for their assessment.

How it works

From your wrist to a responder's screen in four steps

01

Keep your Health Profile current.

Your Emergency Profile is built from it, so what a responder sees is only as good as what you keep there.

02

Wear the Sleeve.

It slides over the band of the watch you already wear, including the one the mobile app syncs for your HealthScore.

03

A responder scans it.

Any phone camera reads the code. No app, no login, no waiting for a family member to arrive with a folder.

04

They see your Emergency Profile.

Who you are, your allergies and alerts, your primary emergency contact, your care team, and your HealthScores. It's what you chose to share, as of today, not your full record.

Provided in partnership with MyID

A partner who has done emergency ID for years

The sleeve comes from our partnership with MyID, whose emergency identification products have been worn by people with chronic conditions, allergies, and complex medical histories for years. HealthScores.ai adds the living profile behind the code.

The sleeve is the first of these, and we plan to add more ways to carry your code, like scannable wristbands and wallet cards, as members tell us what fits their lives.

What the sleeve QR code opens

An Emergency Profile first responders can quickly see

The sleeve QR code doesn't open your full Health Profile. It opens your Emergency Profile, a separate view built from it with only what a responder needs: who you are, who to call, your allergies and alerts, your care team, and your HealthScores breakdown.

The rest of your records stay in the app. To share at a visit you're present for, use the SMART Health Link on your shareable Health Profile document instead.

The HealthScores.ai Sleeve

Your emergency health profile, on your wrist

This silicone sleeve slides over your watch band. The HealthScores.ai mark faces out, while an Emergency Health Profile Share QR code sits underneath so responders see your allergies and medications the moment they scan it. It pairs directly with the emergency profile you already manage in the app, ensuring the information you choose to share stays current. Update your profile, and your sleeve is automatically up to date.

Sleeve questions

  • Do responders need an app to scan it?

No. Any phone camera reads the code.

  • What if I stop using HealthScores.ai?

The code points to your profile, so if your membership ends, it won't open your information anymore.

  • Can I wear the Sleeve without a smartwatch?

Yes. It fits over most standard watch bands and wristbands.

  • How do I get a Sleeve?

The Sleeve is $20, one-time, made in partnership with MyID. Request access to HealthScores.ai, and our team will walk you through the ordering process.

  • Is the sleeve a medical alert device?

No. The sleeve carries a QR code. It does not detect a fall, call for help, or notify anyone. In an emergency, call 911. The sleeve makes your information available once help arrives.

Make sure the right people know your history, even when you cannot tell them.

Ready when you need it most.
