What is a FHIR bundle?

A FHIR bundle (pronounced “fire bundle”) is simply a digital package containing copies of your medical information.

Think of it like an electronic folder that may include:

Your medication
Allergies
Medical conditions
Test results
Immunizations
Recent visits
Doctors and care team
Insurance information

The downloaded file may look like computer code or have a name ending in .json or .zip . That is normal. The formatting is standard, allowing the HealthScores.ai application to read and organize it for you. You will not need to do anything. It will be downloaded and stored on your personal Google Drive.

A few important tips

Download it only to your own phone, tablet, or computer
Do not change the file’s name or contents.
Keep it private because it contains personal health information.
Do not email it unless you trust the recipient and the email method is secure.

In everyday language: You are downloading a portable electronic copy of your medical record so it can be securely connected to the HealthScores.ai application to share your information during a scheduled office visit or in an emergency.

We signed the CMS-aligned network pledge to "Kill the Clipboard". Here is what it means.

On July 27, 2026, Eric Stephens, Founder and CTO of zPaper and HealthScores.ai and Judson Bruno, Product Manager traveled to Washington, D.C. to attended an event held by the Centers for Medicare & Medicaid Services (CMS) to formally sign their pledge on behalf of zPaper, the company behind HealthScores.ai. documented pledge that validates the alignment of zPaper with CMS's "Kill the Clipboard" initiative. They were joined by many other healthcare companies who are collectively helping to support this important initiative.

While they were there, they signed a pledge on behalf of zPaper, the company behind HealthScores.ai.

We want to be clear about what that is and what it is not. The pledge is voluntary and open to any company that wants to sign it. Signing it does not make us part of CMS or any government program. It does not mean CMS approves or endorses HealthScores.ai. It is a promise we made about how we build.

Here is the promise, to which we signed:

HealthScores.ai publicly commits to giving members clear control over their health information, obtaining informed consent, limiting data use and disclosure, protecting information with appropriate security safeguards, preserving the source of medical information, and providing transparent options to access, share, disconnect, or delete information.

That is written for the industry. Here is the plain version.

Your health history is probably spread across several places. A hospital has part of it. Your primary care office has another part. A specialist you saw two years ago has a third. Each one gives you a portal with its own login, and each portal shows you only its own piece. Nobody holds the whole thing. That includes you.

So every time you sit down at a new front desk, you write the same list out again from memory. Your medications. Your allergies. The surgery you had. The name of the doctor who ordered the test.

Through its "Kill the Clipboard" initiative, CMS wants to make starting a doctor’s visit as easy as scanning a code on your phone. Instead of sitting in the waiting room filling out paper forms by hand, patients can share their info in seconds. At the event, several companies showed off the tech that makes this hassle-free check-in possible.

This is the work HealthScores.ai was built for. We bring your records, your portal information, and readings from devices like a watch or a blood pressure cuff into one place. We turn that history into a Health Score on a scale of 0 to 1000, so your family can look at it and understand where you stand.

Your health profile lives in a file on your own phone or computer. Nothing leaves it until you decide who gets it.

When you want to share, you build a SMART Health Link. You choose exactly what goes in it and exactly who receives it. That might be your spouse, your daughter, or the front desk at a new specialist's office. There is a second code you can use as well. It carries the same record in a format your care team's system is built to accept, so their staff can bring it into their system instead of typing it in from a form.

Signing a pledge is the easy part. The work is what comes next, and CMS will set more milestones for companies that signed. We will keep you posted on ours.

Kill the Clipboard: What It Means When You Are the One Managing Care

You know the folder. It might be an actual folder, or a note on your phone, or a photo of a pill bottle you took in a pharmacy parking lot two years ago because you knew you would need it again.

If you are managing care for a parent, a spouse, or a child, you have become an unpaid records department. You know the dosages. You know which specialist ordered which test and roughly when. You know the answer to "any changes since your last visit," because you are the change log. And every time you walk into a new office, someone hands you a clipboard and asks you to produce all of it from memory, accurately, in ten minutes, in a waiting room.

That is exactly what CMS is trying to end.

One honest note. This is a voluntary commitment, not a law. No hospital is required to participate, and adoption is uneven. It is getting better, and being clear about where it stands today is more useful to you than a promise that everything is already fixed.

In 2025, the Centers for Medicare and Medicaid Services launched an initiative called Kill the Clipboard. It asks technology companies to commit to something specific: let people retrieve their own health records and share them with a provider digitally, through a QR code, using shared standards that any health system can read.

Roughly sixty organizations signed on. Eleven health systems agreed to accept patient information shared this way, which is the part that makes it real. A digital record only helps if the office on the other side can open it.

Instead of trying to remember specific medical history details to provide for intake documents, you show a qr code to make it digitally accessible, when you need it.

A SMART Health Link is a secure link that carries a set of health data members choose to make digitally available. When a provider scans the QR code , the information becomes available to them as digital document and structured FHIR data which can be pulled into an EHR, not a paper document or photo they have to manually pull information from. Health profile information a patient can be shareable includes (but not limted to) current medications, allergies, active conditions, and emergency contacts.

The information stays under the control of the person it belongs to so that they decide what to share and with whom. It is not a database someone else operates on your behalf.

Most days the clipboard costs you fifteen minutes and some anxiety about whether you got the dosage right.

Then there is the day you follow an ambulance to an emergency room.

In that room, the person you care for may not be able to answer questions. You may be too rattled to recall a medication list you know perfectly well on an ordinary afternoon. Whatever they decided in advance about their own care is sitting in a drawer at home. The people treating them are working without the context you spent years assembling.

A scan makes that context available right then. Not a guarantee of a particular outcome, because nobody can promise that. What it does is put the information in the room instead of in your folder at home.

The moment this matters most

Most health apps assume you are managing one record, your own. That is not the situation you are in.

You have your mother's records and your own. Maybe a child's too. Separate logins, separate portals, separate paper trails, and every one of them expects you to be a different person.

Ask your providers whether they accept digital records through QR codes or SMART Health Links. Enough people asking is what moves adoption faster than any pledge does.

Start consolidating. Even before every office participates, having records in one place means you are not rebuilding history from scratch at each new appointment.

And keep the folder for now. The system is changing, not changed. There is no reason to throw away the thing that has been working while the rest catches up.

What the CMS Kill the Clipboard pledge means for employers

Most benefits leaders did not notice when this happened, and that is fair, because it did not arrive as a regulation.

In July 2025, at a White House event, the Centers for Medicare and Medicaid Services announced commitments from roughly sixty healthcare and technology organizations around a shared interoperability framework. Apple, Google, Samsung, CVS Health, and UnitedHealth Group were among them. One piece of that framework got a name memorable enough to travel on its own: Kill the Clipboard.

The commitment is narrow and specific. Companies that sign agree to let patients retrieve their own health records from CMS Aligned Networks or personal health record apps and share them with providers through QR codes or SMART Health Links, using FHIR bundles, which is the open standard health systems use to move clinical data. Where possible, they return visit records to the patient in the same format. CMS states the goal directly: eliminate the need for patients to repeatedly recall and write out their medical history.

zPaper signed that pledge, and HealthScores.ai is the platform that delivers on it.

The clipboard looks like a patient inconvenience. Follow the cost and it stops looking that way.

An employee filling out an intake form from memory is producing an incomplete record. Incomplete records lead to repeated tests, missed interactions, and appointments that end without resolution and get scheduled again. Each of those is a claim. None of them appear in your reporting as a data problem, because they appear as utilization.

Then there is the part that shows up in productivity rather than claims. A large share of the workforce is coordinating care for someone else, a parent, a spouse, a child with a chronic condition. That coordination happens during business hours, because that is when medical offices are open. It looks like a two hour absence for an appointment, and underneath it is an hour on the phone requesting records, another hour filling forms with information the employee is not confident about, and a follow up appointment because something was missing the first time.

Be clear-eyed about this, because it is not a mandate and treating it as one will make you look uninformed in a vendor conversation.

There is no federal requirement here. No fines, no enforcement deadline, no compliance obligation attached to signing. CMS set a target of the first quarter of 2026 for early progress, but that is a goal, not a rule. Any vendor telling you they are "CMS compliant" because of this pledge is describing something that does not exist.

What the pledge does create is a shared standard and public accountability. Sixty organizations agreeing on how records should move is more useful than any one of them building a proprietary version. Eleven health systems committed to accepting patient information shared this way at launch, which is the piece that determines whether any of it works in practice. A QR code is only useful if a provider can scan it and read what comes back.

Momentum has continued since launch. Imprivata joined in January 2026, and Samsung and b.well announced partnership work around the initiative at HIMSS in March. This is an active movement rather than a one-day announcement.

Where HealthScores.ai fits

HealthScores.ai brings a member's records, connected device data, and health signals into one place, and it generates a SMART Health Link so those records can be made available to a provider with a scan.

The Care Circle is the part that speaks directly to a benefits population. A member can manage records for the people they care for, so the employee coordinating a parent's specialist visit and their own annual physical is working from one dashboard rather than a separate login and a separate paper trail for every person they are responsible for.
